1.1 This policy explains when and why Improve Personal Training (Improve PT) may collect data about clients, how it is used and your rights in relation to it, in compliance with the General Data
Protection Regulation (GDPR) which will came into effect on 25th May 2018.
1.2 Improve PT may collect, use and store your data as described in this policy and as described when it is collected from you.
2.1 Improve Personal Training is Andy Coyne, trading as Improve Personal Training (Improve PT)
- your title, full name, date of birth/ age,
- your email address and telephone number,
- your medical history, blood group and doctor's details,
- your emergency contact's name, their relationship to you and phone number
- details of your lifestyle, exercise habits, exercise history and exercise preferences,
- your height, weight, BMI and anthropomorphic body measurements.
- This data is generally collected through the completion of three forms: General questionnaire, Physical Activity Readiness Questionnaire (PAR-Q) and Informed
Consent.
4. Why does Improve PT collect this data?
4.1. Improve PT collects this data for the purposes of providing Personal Training to you. This data is needed to ensure your training
programmes are appropriate to you and best suited to achieving your goals.
4.2 Data such as telephone number and email address is collected for the purposes of contacting clients in relation to changes to planned sessions
and planning future training.
4.3 Emergency contact details are collected in the very rare instances that clients are injured and need someone contacting for them.
5 How does Improve PT protect your data?
5.1 Improve PT will never transfer your personal data to any third party without your prior permission, unless necessary for the purposes of taking legal or insurance
advice.
5.2 Improve PT has implemented generally accepted standards of technology and operational security in order to protect clients' data. Please note however, that where you are transmitting information to Improve PT over the internet, this can never be guaranteed to be 100% secure.
5.3 Improve PT will notify you promptly of any breach of clients' personal data which might expose you to serious
risk.
6. Who else has access to information provided to Improve PT?
6.1 Improve PT will never sell your personal data. Improve PT does not share personal data with any third party without the prior consent of clients (which you
are free to withhold), except where Improve PT is required to do so by the law or for the purposes of taking legal or insurance advice.
7 How long do we keep your information?
7.1 Improve PT will keep your information as long as you are a client and then for as long afterwards as is in Improve PT's legitimate interest
and in order to comply with legal and insurance requirements. Improve PT will review data every year to ensure it is still needed.
7.2 Data will be securely destroyed once it is no longer required.
8 Your rights
8.1 You have rights under GDPR:
- to access your personal data,
- to be provided with information about how your personal data is processed,
- to have your personal data corrected,
- to have you personal data erased in certain circumstances,
- to object to or restrict how your personal data is processed,
- to have your personal data transferred to yourself or to another party in certain circumstances.
